Complete the additional required forms.
- STUDENT PHYSICAL FORM
- It is mandatory that each new student, 3rd, 5th and 7th grade student provide a physical examination form completed by the child’s physician.
- EMERGENCY CARE PLAN
- fill out the form if applicable
- MEDICATION AUTHORIZATION
- Return completed form to office
- SCHOOL READINESS TOILET LEARNING PROTOCOL
- students entering 3K or 4K (required signature)
